TRANSITIONS takes great pride in providing our families with the highest quality caregivers. We strive to give our families compassionate, reliable, dependable care for their loved ones. All of our providers are required to be up to date with their certifications including Article 9, CPR and First Aid.  We also do various background checks on all of our employees:

  • Child Protective Service (CPS) Check
  • Reference Check
  • Driving Record Check
  • *Fingerprint Clearance Card/Criminal Background Check from the State of Arizona Department of Public Safety*

TRANSITIONS matches the personality traits of the staff to those of our consumers to provide the best fellowship and camaraderie for the support of each consumer. Before a caregiver starts with a new family, we recommend that the caregiver interviews with the family since they are the ultimate decision maker in this process.  IDLA Homes:

We are currently looking for responsible and patient individuals to provide at-home care to developmentally disabled adults. The following positions are available:

Duties include:

  • Supporting independence with daily tasks
  • Creating a positive environment
  • Medication administration
  • Paperwork
  • Transportation
  • Light housekeeping
  • Working closely with the House Coordinator and other Staff for a positive team approach

Home and Community Based Service Caregivers

Part-time weekdays and/or weekends

Home & Community Based Services (HCBS) is a service and/or care that is provided in a consumer’s house or in the community. Transitions offers Respite (RSP), Habilitation (HAH), and Attendant Care (ATC).  Transportation may be needed, candidates must have a good driving record & a vehicle. POSITION IS PART-TIME ONLY.

Respite provides short-term care to temporarily relieve the primary caregiver(s) for some well-deserved time off. Transportation may be needed.

Habilitation is working on life skill goals with individuals who can physically or cognitively learn to do the goals by themselves. Transportation may be needed. Monthly paperwork tracking the individual’s progress required.

Attendant Care provides a qualified attendant to assist an individual to attain or maintain safe and healthy living conditions within the member’s home.

 Current Needs: 

  • We always have a need for after school hours to be covered: 3:00PM – 9:00PM. Transportation is needed for most.
  • Some families need caregivers to be on-call for RSP for nights such as date night or work functions on Friday or Saturday evenings.
